It sounds like your commission settings need to be as follows:

Comm type: 0 (Money)
Base Comm: 4
Comm lots: 1 (Per trade)

That's correct - MT4 will automatically calculate the commission during the back-test.
Yes, you will need generate the settings for your broker via the TickstoryInfoExpert EA (instructions are in the "Help" tab in the 'Export to MT4' dialog window).
